Are you sure the care is ttyS4?  Check /var/log/messages after the
card is inserted to see what port is assigned to it.

I'm not familiar with RH dialer or kppp (I do it the "hard" way and
manually create the ppp config files), but if the menu only goes up
to ttyS3 (and your card is really ttyS4), you could try and disable
an unused COM port (or an IR port) in your BIOS, which may make the 
modem get assigned a lower number.
