How much space is available in the filesystem containing your cache dir
(/usr/local/squid/cache)?

Try removing "no_cache deny QUERY".  Really most webservers handle this
on their own quite well,  squid doesn't cache pages the servers don't
think should be cached.  It is a pretty ineffective rule anyway as it
won't match ASP, PHP, PL, pages, etc....
