What shows up in /proc/scsi/scsi?

What SCSI controller are you using?  Can you "modprobe -r {driver}"?
If so what happens if you "modprobe {driver}"? (Watch dmesg as you
try these).

Anything in /var/log/messages that looks related?

Change your cableing recently?
