I recently had a problem with a USB device with a bad cable, which
caused all kinds of problems in the kernel, requiring a reboot.  

Since  I replaced the cable, the device works fine, ... except now I
have to modprobe the driver before the device will work!  It used to
hotplug fine before (no manual modprobe needed).

A co-worker wanted to try the cable himself, and now he also has the
same problem on his computer - good cable now requires a modprobe.

It's acting like USB is "remembering" the bad device/cable.  Is there a
cache file somewhere for USB that I can wipe out to eliminate the need
for the modprobe???  This is weird ...
