[KLUG Members] A [simple?] question

Despite the subject, this is not a "trick" question. I seem to have 
encountered an organization that creates and sells a lot of software
that does not appear to know the answer, and I want to cosult the
collective knowledge of the group as part of  my dialogue with that
organization. Here it is:

On UNIX systems generally, what determines the permission settings for 
a file that is newly created by a program?

I'm looking for what I beleive is the overwhelmingly common answer to 
this question, although I understand that there may be exceptions.
