It looks to me that sendmail on SuSE 10 has libmilter compiled in.
The include files are there, along with the .a library.

Take a look at:
/usr/share/doc/packages/sendmail/README.libmilter

"Starting with 8.13 sendmail is compiled by default with support for
the milter API."
