Please move this thread to the speakers@ list (I'll double post this one).

Even Book TV (my favorite channel) has to use a what looks like a boom mic to
hear questions from the audience.  It is a mic on the end of a long stick they
shove in the face of the audience member asking the question.  This of course
requires an operator.
