After some more digging, I found this is not an exim issue at all but it
has to do with the mail client.  Outlook and Evolution both insert the
host name in the header like this  (helo=[shackman5.local]) and those
clients can send to aol addresses without a problem.  Thunderbird,
however, inserts the originating host ip like this (helo=[10.3.1.19])
and that seems to generate the aol error.

Now, does anybody know where Thunderbird is pulling this from?  It is
not hostname and it is not a dns lookup.
