There are some development environments that require it.
Customarily, I put something into the source code that shows my name and the
date that the code was turned over into production. It is not extensive or
obtrusive, and has no contact information. Experience shows that this is usually
enough to identify things and place them in some historical perspective.

I'm sure such a term exists, but I don't really know. I've always called it a
signature.
