It appears from your log that you have authorization set to PAP or CHAP.
I dialed your ISP's modem with minicom, and get a "login:" prompt right off.
That implies to me that your ISP uses standard text based authorization, and
NOT PAP/CHAP. Try changing your authorization method, and then select the
Standard Login Script. If that doesn't work, send the new log.
