Nothing in dmesg?  Maybe lost IRQs?  Is the card seated all the way?

DO you see errors on the output of ifconfig when the interface is
"stuck"?

After that,  I'd really suspect the card is just fried.  Especially if
your box runs all the time or has poor ventilation.
